Join us on January 30, 2025, from 1:00 PM - 3:00 PM EST for an exclusive virtual workshop presented in partnership with Probari, focusing on the Indiana PathWays Care Coordinator Role. This event qualifies for up to 1.5 CEUs!

In this workshop session, we will dive deeper into the role of Care and Service Coordinators within the PathWays program, reviewing key aspects such as membership assessment and benefits coordination.

Presentation Topics:

  • PathWays for Aging NH Care Coordinators
    • Current state and goals of the program
    • Coordinator objectives and minimum requirements
    • Coordinator job duties

Last year, Dr. Kathleen Unroe and Russ Evans from Probari shared insights on how to best prepare for integrating these roles into your facility. This year, we’ll revisit the progress made and provide further guidance to ensure successful integration into your daily operations.
